Sanction Request Form
Host clubs are required to submit a Sanction Request Form for approval from Football Queensland and league management for any game involving QSL players/clubs that takes place outside regular QSL fixtures. Please consider the following when completing the Sanction Request Form:
1. Requests will only be considered if received by Football
2. The Sanction Request Form must be completed in full to be considered by league management

Clubs are advised to take into account the following rules when completing a Fixture Alteration Request Form:
1. Requests will only be considered if received by Football
2. Fixtures can not be changed to a rest week or catch up week.
3. The alternate fixture requested must be within 1 week of the original fixture date.
4. The Fixture Alteration Request Form must be completed in full by the applying club to be considered and must contain the club representatives signature prior to submission for consideration by league management.
5. Changes are not approved until official Fixture Alteration Advice has been received from league management.
